Obverse: MEDAL OF HONOR above applied symbols for various branches of learning, and below ST. PETER'S ACADEMY ROME N.Y. Reverse: AWARDED TO Mary S. Olmstead FOR PROFISIENCY ON PIANO AND ORGAN EUPHRASIA SUP. In original round mock-ivory case, lined in blue satin.
